This membership is for 3 or more individuals. Can have up to 8 individuals on a family account. If you need to add more than 8 individuals to your account an additional fee will be charged. Individuals over the age of 18 must show proof of address; such as Governmental ID & Utility Bills. Packaging slips from distributors will not be accepted. This Membership is only for Students who are attending High School or attending College. This membership is also for individuals 13 and Older. This is based on a month to month rate. If member wishes to cancel they must give a 30 day written & verbal notice. Students under the age of 18 must have parent/guardian signed permission. Must show proof of enrollment of school/college. Did you know that some Insurance Plans will pay for a gym/fitness membership. Check with your insurance provider and see if your insurance has a gym/fitness plan. Fitness Forum Sports & Wellness will allow Members to cancel on Medical Cancellation. Clients must provide a Written Letter from their doctor/physician explaining why they can no longer workout in order for them to use a Medical Cancellation.
